- Select Directorate of Immigration Services
- Select passport application
- Choose the passport owner (yourself or your child)
- Choose the passport type (ordinary or diplomatic)
- Choose the type of application (mutilated passport or lost passport)
- Fill in the required details and download application form
- Upload required documents
- Book an appointment with the Immigration Department in the County
- Attend your appointment with the required documents.
Documents required for lost passport:
- Passport application form
- Applicants ID copy
- Recommender’s ID copy
- Police abstract
- Affidavit for replacement of lost or damaged passport affidavit for lost or damaged passport by an adult
- Letter explaining circumstance
- Passport photos (3)
