ABOUT THIS DOCUMENT
This affidavit is required by NTSA on application for replacement of a lost or damaged
number plate.
REQUIREMENTS
• Motor vehicle registration number.
PROCESS
• Fill in this affidavit.
• Sign before a Commissioner for oaths, Notary Public, Magistrate or Consular officer.
• Obtain a police abstract
• Take motor vehicle for tape lift
• Submit the affidavit together with the police abstract and tape lift on NTSA TIMS.

NB: You MUST appear before a commissioner for oaths near you to stamp this affidavit eg (Advocate, Magistrate, Consular Officer or Notary Public). A commissioning charge will apply.
